Do you support Hostgators Website Builder platform?

I'm trying to use Jotform for my "Serve of Use" and "Privacy" form. My form asks the viewer to ckmark their acceptance. How do I receive it a copy of this for my records?

If you do support Website Builder please guide me do I add a button element, would these forms be embeded?  Website builder will not give me the option to embed this into the footer only into the page. (Hope I have this right) Please let me know. Here is an image to assist you.

I am not 100% familiar with Hostgators website builder but if they allow for inserting HTML in any way, you can add your form using its iFrame code:
