Why is my embedded form displaying distorted?

    Hey guys, my jot form used to look perfect with the webpage header at the top and the form underneath. Now for some reason the web page header as at the bottom of the page. Any idea why?

    I have checked your webpage and it seems like the form's code has been indeed placed at the top of the page.

    If would recommend you to check the source code of your page and make sure that the form's code has been placed in the desired section. 

    Also, if you're opening the form with a pop-up code, you may need to check that there is not any conflict there that may be causing this.
