I recently had an issue with a form that contained a password box - you suspended our service temporarily, removed the form and gave us our access back again.
Unfortunately you are still blocking our work IP address
You sent me an email this morning re account over quota and I can't access your website to upgrade (I'm sending this from my phone!!
I've emailed you about this several times, but have had no reply.
I will have to upgrade to a paid account with someone else if you can't get back to me.

Upon checking, your IP is not blocked on our site. So you should be able to login to your account, although you cannot embed your form since it is currently suspended for over quota.
Can you please specify your current IP address for us to investigate further?

The IP you have is not the one that's blacklisted - that's the ip address of my mobile phone.
The office IP that's blacklisted is 22.214.171.124

I have checked and was not able to find that IP associated to any of the accounts in JotForm. However, I have added the IP to the whitelist.
Please let us know if that resolved the issue!
