I see that you are trying to iframe embed a location you set in Google Maps into your form using the Text (HTML) field.

Upon checking your form from a clone version, the location is defaulting to California because that is what you set in the iframe's src attribute.

    <iframe style="border: 0;" src="https://www.google.com/maps/embed?pb=!1m18!1m12!1m3!1d3304.595484913292!2d-118.42514348478416!3d34.07988248059843!2m3!1f0!2f0!3f0!3m2!1i1024!2i768!4f13.1!3m3!1m2!1s0x80c2bc0a3c44c8cf%3A0x202f1024a3625ac9!2sSunset+Blvd%2C+Los+Angeles%2C+CA%2C+USA!5e0!3m2!1sen!2sph!4v1491472585308" width="500" height="350" frameborder="0"></iframe>

I noticed that the address you're trying to show is — The Benton Convention Center 301 West 5th Street, Winston-Salem, NC 27101

I got the embed code of that location from Google Maps for you.

    <iframe src="https://www.google.com/maps/embed?pb=!1m18!1m12!1m3!1d3223.717004991937!2d-80.25008168472964!3d36.10038598009869!2m3!1f0!2f0!3f0!3m2!1i1024!2i768!4f13.1!3m3!1m2!1s0x8853ae43cea1a2d7%3A0x474066c4749ecfea!2sBenton+Convention+Center!5e0!3m2!1sen!2sph!4v1516567961448" width="600" height="450" frameborder="0" style="border:0" allowfullscreen></iframe>

I hope this helps. If you have other questions or concerns, please feel free to let us know anytime.
