How can I make this form work on the iPhone? (not working - works on my desktop, iPad, and Android though)

I am super new at this and still learning.  I just made a new form "HFAF Employment Application".  It works fine on my desktop, iPad, and Android, but it won't work on my iPhone 5, 6, or 7.  Any ideas, or am I just a dummy?  Thank you - Peace,

Looks like your website it's not fully responsive on mobile devices. I tested out just the form url: https://form.jotform.com/80207452184149  on iphone and it worked fine on my case.

I would suggest you to use the forms URL. You can modify the url as you want, please check the instructions on the link below:
