How do you insert a scrollbar for a long form?

You should not need to set one unless embeding the form on a frame that has preset sizes.
One good suggestion for embeding the form is using the iFrame code. Here is a guide on how to find such code. You can edit the width and the height of the iFrame and also the scrolling option (no, yes, auto). The code should look something like this:
<iframe allowtransparency="true" src="//form.jotform.co/form/22766385750866" frameborder="0" style="width:100%; height:465px; border:none;" scrolling="no">
