Hi, I'm trying to create a form where based off of the employee # someone uses it auto-populates the rest of their information such as name, work area, and shift. Almost like a v-lookup in excel but for Jotform. Is this at all possible because I have over 5,000 employees?

If you have this information in a spreadsheet you are in luck.

We have a spreadsheet to form widget:

You can read the guide here:

Aubreybourke - Thank you for the speedy response this has been a tremendous help! My only question now is that I have this can I actually have 2 of these widgets on 1 form doing the same thing from different excel docs without auto-populating both name fields?

The problem is that the labels of the fields need to be the same as the corresponding column in the spreadsheet.

That's why in your demo the "Name" field is populated twice.

If your second spreadsheet column was "Name2" and the field label was "Name2" then this wouldn't happen.
