Form not loading properly on Squarespace site

I've followed all instructions as to creation of the form and embedding on the Squarespace website but there are loading issues and I can't figure out why.

Sometimes it will load but only after refreshing the browser window. Other times, and on other computers, the form is truncated and doesn't load at all (see screenshot). Please help!

I just look at your site, and the form loaded just fine for me. If the issue continues, please try embedding your Iframe code without the script part, example:
