How to add multiple fields with the same heading?

    Hello,

    As you can see from my screenshot, I would prefer to build something that looks like the 'Address' form element in that it has several fields under the same heading. Is it possible, however, to a) either build my own so that I can add more fields? Or b) add more sublabels so that I can include one more question for this form. The question I am trying to add is: "Relationship to you:"

    Thank you for your input!
    I believe I tried that but I wasn’t able to get the form calculator to recognize that each field was a different response. Instead, when I ‘Add Field’ on the calculator to add them all together, the field is grayed-out, not allowing me to select it as a distinct value. Is there a way around this?

    That is true, fields in the Multiple text field cannot be used in calculations as a separate.

    So what you would like to achieve can only be done with regular fields and then aligning them with field properties options (width, shrinking) and CSS.

    For example, I used text fields for each of your field:

    So you could later use each field in calculations.
