Why there is no submit button on the form?

I have tried everything to get the submit button to appear on my form.

The whole website is https.

I have tried it with and without the back button (which does appear).

I have tried checking the box that is supposed to go to the next page after all boxes are filled in - does not work.

I have filled the boxes completely everytime with actual correct info but still no submit button.

What am I doing wrong?
