How can I fix the alignment of my questions?

I want to fix the alignmet of my questions. How can I do it?

 

I have checked your form in multiple browser but I could not reproduce the alignment issue.

I was not able to reproduce the issue in Google Chrome.

Are you having this issue in your mobile phone, since I am checking it from PC browser and it is fine here.

Please confirm.
