Your question is incomplete, but if you want to style the form, you should be able to do it in the Designer. You are in a mixed layout which means you'll see the card and standard layout. For card layout, you are limited to style the form using the styling options in the designer. For the standard layout, you can customize the form to anything you want, in this case by custom CSS codes.
