How to create a form like the link below

I am looking to create a form that will end up looking like the form attached below. I would like the output to be a final PDF of the filled out form.

How is this possible in the form builder? I understand and have a handle on the adobe docusign. I just dont know how to create the form!

You can also re-create it. The widgets used in that form are Narrative Fields and Adobe Sign. You have to connect the Adobe Sign widget with your Adobe Document Cloud Account.
