Violated TERMS OF USE...Not sure how

    I received notice that I was in violation of the terms of use.  I read them and cannot find where I went wrong or violated any of the terms?  I do, however, understand that JOTFORM reserves the right to cancel or suspend service for any reason they deem fit.  So I would like to know the following:

    1. What did I do wrong?

    2. Can it be corrected?

    I assure you that any violation was an oversight on my part and was in no way deliberate of malicious.  I am using the forms to get feedback from my customers regarding their needs.  I posted the links on my website, but can email them the surveys if that will remeedy the problem?

    Your account was suspended because you have a form which asks for email and password. Such a field combination (email/pass) are used mostly for phishing.

    May we ask the purpose of this forms? If you can provide us a good reason of this form we can however re-activate your account.
