I can't log in to my account. It says "Login Failed - SUSPENDED"

    I can't log in to my account. It says "Login Failed - SUSPENDED" what is this?

    Your account (hypesupply) is currently suspended. This form (https://www.jotform.us/form/80238433284153) have fields that are asking "passwords" and "credit card information" using regular text fields.

    Please take note that collecting credit card details using regular text fields and asking for login credentials are not allowed here in JotForm. I suggest you check our terms of use.

    We need to delete the guilty fields so that we can reactivate your account. If that is okay with you, let us know.

    If you need to collect payments from your users, please use one of our payment processors. Here's a guide: Setting-Up-Your-First-Order-Form. You may also capture the card details and charge the user at a later date. Here's a guide: How-to-Enable-Payment-Authorization.
