We are going live on Monday and need to see who is subscribing to our service. But, we are not receiving any emails once some one fills out the form and submits it. Please help as soon as possible.
It was working the other day and then it stopped. I am sure I fat-fingered something, but I can't find it.

I checked your form and the problem is that you set a condition, so the email will be sent only if the condition is true.
But, further, the problem is that that condition is set on a Text field, and that cannot work:
Since the Text field is not fillable field.
I would advise you to remove that condition since there is no point to have it.
Then the emailing should work fine.
