I checked the logs on your account and I saw that you created two forms on Wed, 24 Jan 18.
Those forms are located inside of My Forms folder on your account.
Are you unable to find some other form? Do you have an ID of that form? If so, provide us the ID and we will find the form. Maybe you created your form on some other account or you were not logged in, so the form was created on the guest account.
Please note that we can not find the form without the form ID or with the link of a form.

If the test run of my project goes OK I’ll need to upgrade to the $39 per month level. Can that be done fast?
