User of form can not sumbmit form

Hello

I am still battling to have a working form, it works on preview and my Pc however when I provide link to other users the from will not submit and gets stuck in a loop asking for one of the service boxes to be ticked. If you tick it and re-submit the from circles around asking for the next option to be ticked and so on. If you pre-tick all boxes and submitted the from hands and the name fields become highlighted in yellow

I did a test submission to your form and it was successful. I was not able to replicate the issue you're describing. Here's a screen capture of my test submission:

Please test it again and if the issue still persists, please try to provide more details on how can we replicate the issue. Also, try using different internet browser just to test it.
