Why is "textbox_sample" show up in my boxes?

Why is "textbox_sample" showing up in my boxes?  Did I do something that can be undone?  See screenshot attached.

I checked your form on the browser but I was not able to reproduced the issue you described.

From the screenshot image you shared, that appears to be the Preview screen of the form when in Form Builder.

That is just in preview, but the live form https://www.jotform.us/form/73485684955172 will not have that auto fill data on it.
