I have a domain name to host my form jotform: www.cnio-haiti.org (my provider and constructor is "Weebly").
I created a sub-domain "forms.cnio-haiti.org" for my form jotform (to replace the link provided by jotform: https://form.jotform.co/JosemarStvictor/test-orientation-metyepam).
In my searches, I found your page: https://www.jotform.com/help/144-How-to-use-Custom-Domains-with-Jotform
But I do not master the technical language "DNS", "CNAME", etc ... SO, PLEASE MAKE IT FOR ME, ie allow me to host my form jotform via my subdomain " forms.cnio-haiti.org ".

Unfortunately it is not possible for us to go to that extent since we are not the service provider of your website/domain.
I understand you are not capable to with the user guide, but I recommend that you seek for the assistance from your web/domain host service provider instead. They should be the one capable of assisting on this process.
