Single & Multiple questions to display on the same form

    In the same form I want to display the questions in two different ways- 

    1. Single questions in one screen 

    2.Multiple questions in one screen.

    Please see the screenshots.

     

    What is the solution?

     

    If this is not possible then can you tell me how can I combine those multiple questions as one question?
