I checked the form from the link my colleague provided and it is showing on my end.  What is the URL being used that shows the form is disabled?  Does it show the same on your end from this URL?

I was able to reproduce the issue with your form but not with my clone:

Try removing the form expiration date and set the form to "Enabled".  Test the form to see if submissions go through.  If it does, enable the expiration date and submission limit.  If there error returns, let us know and we will take another look.

You can also try cloning my copy to see if that works:

It was a very odd error.  If it does happen again, please let us know and we will look into this further.
