I didn't think I had any violations on my forms

    Please unsuspend mine too?   I didn't think I had any violations on my forms?

    I have re-activated your account temporarily. 

    The form (http://www.jotform.us/form/41707760930152) is collecting SSN which violates our terms of use. 

    Please go to your Trash folder and purge the form (http://www.jotform.us/form/41707760930152) to avoid getting suspended again. The form name is Class Registration Signup Form. Keep in mind that purge forms are not recoverable.

    THANKS!!!   That was an unused form so I guess I didn't realize it was collecting that - my sincerest apologies and thank you for helping to get me going again!
