How do I get remove an item from the form that cannot be identified?

Can you please advise what title or build of #56 on the TransTrack 3.0 form and how do I remove this item from the form?

I checked through your form and I am not quite sure which element you are referring to.  If you wouldn't mind letting us know which field specifically, we can definitely let you know how to remove it.  A screenshot would be most helpful:

I provided a pic in the body of the comment in the previous thread due to I do not have a png pic version to upload and jotform apparently does not allow a word doc to be uploaded that contains a screen shot.

I noticed in the items of my build form for example "see in the conditions for Case Type Transchoice and Provider Select Only" Hide.

#56 does not have a named title so I am not sure what item that is on the form to be removed.

I see which field you are referring to.  It can actually be seen when selecting all fields in the form:

Try click on the left side in between the two fields that are shown.  It should allow for selecting the field:

Hey David that is exactly what I needed. I found the field that was hidden, removed the field, and adjusted the conditions accordingly as it relates to the field that I removed. I have tested and it looks Great! Tthis issue has been resolved.
