Suspended: Jotform template caused my account to be disabled

FYI 

I opened up a Jot form template off of your website titled Student Registration Form.

After opening the template my account got suspended. Your template already had a field to collect drivers license info. I did not add that to the form. 

You might want to remove that before others use that template and suspended
