Why is my account suspended?

My forms have been disabled with no reason, please contact me to fix this problem right away!

Upon checking your account I found that it's suspended by our anti-phishing system. As your form "West Valley Pop UP Movie Vendor Application" has fields like "Tax ID / SSN #" which makes it suspected of being used for phishing.

Creating such forms are against the Terms of Use. I have deleted the suspected form field and activated your account again. Do try to sign in to your JotForm account and get back to us if you have any trouble.
