my form is duplicating itself on the page

Hi, I have this form embeded in a page: https://form.jotformeu.com/80302197432349

But for some reason it is duplicating itself on the page, can you advise please?

Can you check if you have inputted the form twice on your page. In the DOM node it says that there are two rows and each of them has a form inside. 

Please check and remove on of forms from your back-end.

Hi, do you mean on our website? I have deleted all the page contents and re-embedded the code and re-saved the page, but it’s still showing twice.

The form is showing twice in your page due to being embedded twice:

Remove one of the embed codes and the form will only show once.
