I develop on Windows but I have had some feedback that some elements are not working on Safari on a Mac running MacOS High Sierra.
Two widgets do not show at all: Rules and Conditions and a Checkbox one, both near the end of the form. Also, on the widget where they select the product they want, it is not updating the price as it should.
I have other screenshots provided by the person having the problem.

A bit more info on this - all this is via BrowserStack testing:
It DOES show on:
Windows 8 Firefox 30
Windows 8 IE11
Windows 7 IE9
iPad 4th
OS X Mavericks chrome 36
iPhone 6 Plus
iPhone 5S
Samsung Galaxy Tab 4 10.1
Google Nexus 5
Samsung Galaxy S5
It DOES NOT show on:
OS X Yosemite safari 8
OS X El Capitan safari 9.1
OS X High Sierra Safari (via my client)
It PARTLY shows on:
Windows 7 IE8 (Rules do not show, checkbox does)

One of my colleagues tested your form on OS X High Sierra Safari and the widgets are working properly.
Based on the screenshot you provided I saw that you tested the form using browserstack. I also tested the form using with browserstack and the widgets are shown on OS X High Sierra Safari.
However, I was able to replicate the issue when I tested the form with browserstack on OS X Yosemite Safari 8 and OS X El Capitan safari 9.1.
Maybe the issue is related to browserstack. Have you tested the form using an actual device on Yosemite Safari 8 and Yosemite Safari 9.1?

No, I have no access to a Safari Browser on Mac. One of my customers alerted me to the problem as she was on OS X High Sierra Safari - that's what prompted me to investigate further.
I know Macs do sometimes work differently with forms (eg in an address block set to mandatory, on Windows the second line of the address is not made mandatory but on a Mac it is - something I have seen in forms other than JotForm's).

As I mentioned, my colleague tested the form with a real device on OS X High Sierra Safari and the widgets are working properly.
My other colleague also tested your form with a real device on OS X High Sierra Safari and the widgets are working fine on his end as well.
Can you please ask your user to try again?
If the widgets are still not shown, please ask your user to check a console report and take a screenshot of the console error.
How to get a console report from most common browsers?
In case that your form is embedded, provide us the webpage URL.
