Locked out of system

    I have tried 2 times to reset password but it did not work.

    Your account got automatically suspended because the "Class Registration Signup" form is asking for the Driver's License. 


    As per our terms, collecting sensitive information is not allowed here in JotForm. 

    I have removed the Driver's License so I can re-activate your account back in good condition. You can now use your forms and account properly.

    Please try to log in to your account again. If the issue still persists, let us know.
