Forms disappearing off responsive site

Hi!

I used your modern contact form on my responsive site. It does not show up on my iphone SE using safari, it will show up once using Chrome on a desktop then disappears, it will show up once using microsoft explorer then disappears. If I refresh the page on the largest window setting, it comes back, but disappears as soon as I resize the window smaller to mimic tablets and phones.

Once screen is resized, form disappears

It's probably due to javascript conflict since the form isn't executed when viewing on mobile or on a small browser window.

To fix it, re-embed your form using its iframe embed code.
