How can I create a forum that publicizes signatories to my pledge?

I’m creating a pledge for a personal project at my school and I was wondering if it’s possible to replicate this “I sign this pledge” sign up that also publicizes whenever someone has signed it with their name and city. Any advice would be helpful, thank you!
