Ability to create conditions has dissappeared

Hello. On the form I am trying to create, I have fields which require conditions. I was able to put these conditions in and then I don't know why, but the button to enter conditions has now dissapeared from most of the fields i need it in. What have i done to make the button be removed and how do I get it back on all the workshop and presentation options?? I also need to duplicate this form once it is ready and need to make sure I can change conditions in the duplicated forms as well. Thanks for your help!

I have cloned your form to see the problem but once i added the conditions to a field the button to enter conditions can be seen on the field. But if you want to see the button to enter conditions on a field first you must set a condition to that field from "Settings" tab. If you set the conditions and you couldn't see the button to enter conditions anyway there might be a temporary glitch. Here is the image that shows the button to conditions:

And once you clone the form the conditions will be cloned as well. After that, you can change the conditions on the clone form as you want there will be no changes on the original form.
