My form is not fully loading on mobile

Hi, my contact form is only partially loading on mobile.

I'm sorry but I couldn't find the form created by JotForm in the "Wedding" tab of your website. But I found that you used JotForm in "Corporate" tab and I displayed Corporate tab on my phone and desktop but it seems like there is no problem.
