Payment Integration: Create an Initial box to select a product

Im creating a form and need to creat an initial box next to items that customers would like as a form of their service

Is this initial box used to show selection of the option?

Or is the initial box used to capture a quantity?

Please let us know more about the initial box. What is it used for?

In either case you might prefer to use a payment integration. Its basically a shopping cart. You specify your products, list the price, add an image of the product, and so on. And it allows you to take transactions from your clients.
