Yes/No and Comments Section

I am trying to create a Y/N checklist for several questions but with a comments section for each question. I have been playing with widgets and trying to adjust the size of the 'other' option box in a quick element  multiple choice option but I am having no luck. Below is a portion of the document I am trying to create a form for. Any help would be great!!

I inspected your form and I think that I understood what are you trying to archive. It could be possible to increase the size of the Other text box field, with the CSS, but the field would still behave like the text box field, not as text area field. Meaning that the entered text would be shown in the same line and it would not be possible to split it into multiple lines. 

Instead of the other option, I would suggest you to add Text Area fields (Long Text Entry field) below each Y/N checklist.
