Can you make an edit on the form?

I have designed the form but the image and the header is below the form. Im trying to get the header at the top and the image under the header. Then the form.

I can move it but it keeps going to the bottom of the form.

I was able to drag the headers and image to different positions in my copy of the form.  I believe this copy of the form is in the format you are looking for:
