Looking for a mobile subscription template

    Hi,

    I am looking to make a portal for postpaid mobile subscription registration , Which I will be giving out  to several retailers to login and register postpaid customers.

    The details shall be collected and stored in the database and we need to know which retailer is responsible for each registration.

    Before you proceed, it's important to mention that collecting login credentials will violate our Terms of Use. If the retailers you're referring to will be your form respondents, you can setup a form that they can fill out.

    Unfortunately, we won't be able to help you decide which fields to use since this boils down to your needs and preferences. What we can do to help is share some related guides that will help you get started in building a form.

    As soon as you have an account with us, all your submissions will be placed on your SUBMISSIONS PAGE. This will act as your account's database where all your submission data will be stored.

    But, if you aim to push these submission data to your own preferred database, then this would require a developer from your end that will setup the backend script for you. If you're using MySQL, we have an example guide below that shows a basic setup on how this can be done.
