User getting "Page Not Found" error after completing the form and hitting the "Submit" button

Can you please elaborate give more information about the user that has submitted the form? I just tested out your form on the latest version of Google Chrome and I was able to submit the form. Did you receive my test form submission under my name?

If this has happened only with one user, I might suspect that the issue is related with their browser or their internet connection. 

Please let us know so we can investigate the issue furthermore.
