Please note that JotForm is an online form builder that many webmasters use to build a different kind of forms. However, some phishers might use our tool to make forms where they offer jobs, or they request credit card or critical/personal information such as SSN or login credentials.
The form you provided was built by a someone who seems to be using Jotform to scam people. We have a system that detects spammers or scammers, but some forms built with our tool might bypass our automated system detection, so we appreciate your report. The form is now disabled and the account in which the form was created is suspended.
Also can you please let us know from which email address you received an email with the link of that form.

Unfortunately there is nothing we can do about the texts. If you do receive any further texts with new forms, please let us know so we can remove those forms as well.

Similar topic for me. Sometimes I also got weird email and text messages with different proposals to donation money to different banks. For example last text message was from India I checked phone number here https://areaphonecodes.com/india/ This message had propose to donate money to one Indian fond.
