I have successfully connected my asana account and it says it's ready to go. I've submitted two forms and neither one of them have added to asana.

I have checked your integration and I can see you have not mapped fields yet, please note that the fields need to be mapped in order to have the integration working.
I would recommend you to make sure that you have available fields on the task you're selecting, please also note that if you're trying to map custom fields that will work with the premium version of Asana only.
