I have in my form email conditions set up that sends two emails depending if the decision is "approved" or "declined".
At this stage the emails to be sent are not sending.
Any help would be very much appreciated.

I've checked your form and your conditions seem to be right.
But you are trying to send several emails at once, so this emails can be added to the "Bounce List" or may be rejected due to suspected spam.
My recommendation is to switch email notifications to your own SMTP sender for a workaround. Here is "How To":
