Why are we are not able to access our form?

I have also another form <http://form.jotform.com/form/20251711476> but cannot reach it anymore.

The form ID you shared does not belong to the account "PrometheeContact" but a totally different account. 

the reason you can not access the form is because the account has been suspended for having a form "20251711476" used to request for user password information which is a violation of our terms of use.

If you know the owner of the account kindly request them to contact us to have the account activated.
