Can you please provide the submission ID of that submission so I can check. I downloaded your last submission and in my case it was working fine please check the image below:
You can customize how the PDF report will look like. Please follow the instructions on the link below on how to do that:

It seems like you're referring to the PDF fillable version of your form. I have checked it and I can see the issue you're reporting; however, the problem happens since the scale rating field is replaced with a text box since the rating field is not currently supported on PDF fillable forms.
I would recommend you to either remove those fields or replace them with text boxes.
