I have checked your form and it looks like the Adobe Sign Widget is missing Authentication to Adobe.
To Authenticate, please follow the steps in the screenshot below. Try using the Private Mode of your browser (Incognito) to Authenticate because the window to approve the authentication might not appear sometimes if you have already authenticated once.
Do not forget to click Update Widget after successful authentication.
