Hello Kristen - When you say "already created document" I'm assuming it's a PDF form. If that's the case, head over to the page below to convert it to a fillable PDF form.
From there, click the CREATE A FILLABLE PDF FORM button > select the middle option, CONVERT PDF FORM > then fill out your email address and upload your local PDF file.

You will receive an email afterwards stating that "WE ARE IMPORTING YOUR PDF FORMS". This may take some time depending on the complexity of your PDF file and there will always be a chance that the conversion process would fail.
If that should happen, my recommendation is for you to do it the other way around - Create an online JotForm Form, then convert it into a PDF form afterwards.
