Submission info is complete for recipient. Some info is missing in the email sent to me. to be exact the missing info is the address, and the optional services.
the problem with the submit button was solved by using the embed code.Page URL:

You can check first the submissions in the View Submission page. Then check on the sent Email.
If there were differences like some info were not showing in the submission Email, then the Email template must have been manually modified/customized.
This can be fix by re-creating the Email template. That is, add a new Email template to the form. This will ensure all the fields from the form will update the email during submission.
Let us know if this still did fix the issue.
