Why are we not able to integrate with mailchimp?

I'm having issues integrating with Mailchimp for some reason. I added the API Key, which authenticated, but then I was unable to match the fields. I know this list has all user email addresses in the data, so that shouldn't be a blank drop down menu. Please advise how to properly integrate into Mailchimp. Thanks

I have checked your form and screenshot and noted that the form you are trying to integrate with Mailchimp is an empty form as shown below.
