How to create a numeric slider form and get the total amount?

Yes, this is possible. You can use the Numeric Slider widget then use then add some conditional logic to pass the value of the slider widget to the text field.

If you want it to look exactly the same, I think it's possible to copy the layout using Jotform custom CSS styling.

For the payment or total amount, you can pass the total value of the text field to your payment field so when user checked out, they will be redirected to whatever payment processor you used in your form.
