Name change in browser window not correct

I have changed the name of my form from Great Lakes Mall Bridal Registry to PMA Shows Bridal Registry but when you click on it from my website in the browser window it still shows Great Lakes Mall Bridal Registry.  The form is correct but it's showing the wrong name in the browser.
