Authorize.net not linking to my form

Hello I am having some issues with jot and authorized.net at this time:

My first issue is I received a form yesterday via email. Typically I can print it out but the one I received said there is an error and I am unable to do so.  It shows in the body of the email but the attachment is not working.  I need to print out these applications for processing.  

My second issue is the last two applications i received (the one yesterday and the one before that) are not linking to authorize.net.  Ones prior to that I have received confirmation that the payment was successful but I have not received anything for the last few.  I am  not sure if this is something on your end or if I need to contact authorize.net but I really need to get this resolved for our accounting department.

The original content/copy of the email you received can be downloaded on your Submission's Page. You can directly click the "Print" button on your Submissions page

On your Authorize.net widget settings, please try selecting the "Email" in the Customer Email Field since it is Required.
